In-person and virtual appointments
In addition to office appointments, Dr. Gray charges this amount for other professional services you may need, though, she will break down the hourly cost if we work for periods of less than one hour.
Other services include after hours calls requested by patient, report writing, telephone conversations, consulting with other professionals with your permission, preparation of records or treatment summaries, legal representation and the time spent performing any other service you may request. If you become involved in legal proceedings that require Dr. Gray's participation, you will be expected to pay for all her professional time, including preparation and transportation costs, even if she is called to testify by another party.
You, and not your insurance company, are responsible for full payment of fees. You will be provided with a HCFA-1500, also known as a CMS-1500, statement that includes diagnosis code, physician’s NPI, physician’s address and all other information required by insurance companies, which you are responsible for submitting to your insurance company for any reimbursement.
Please note, many insurance companies now have deductibles, so it is important for you to be aware of how much of your deductible you have met prior to seeing Dr. Gray. If you have any questions about your mental health coverage, please call your plan administrator.
Initial evaluation: $310
Short appointment: (20-25 mins): $150
Long Appointment: (45 -50 mins): $250
Medical Marijuana: $150
Initial Suboxone: $310
Suboxone follow-up: $150
